New Patient Checklist

Your first visit will consist of a consultation with one of our oral surgeons. We will take your medical history, review your x-rays and discuss your current oral health condition and needs. The doctor will evaluate this information and discuss a proposed treatment plan tailored to your individual condition.

Referral

Bring the referral from your doctor or other dentist, if applicable.

Current X-Ray

Bring a current x-ray taken within a year.

List of medications

Bring a list of all medications you are currently taking and have been taking recently.

Surgical Appointment Short-Notice Cancellation Policy

If you must cancel your appointment, we respectfully request that you notify us no less than 24 hours prior to your scheduled appointment time. Missed appointments or appointments cancelled without 24-hour notice are subject to a $150.00 fee.
